این مجموعه فرم به عنوان بخشی از پلان پروگرام " افغانستان می‌خواند!" به منظور اندازه گیری عملکردهای این پروگرام توسط موسسه زنان کانادا برای زنان افغانستان تهیه گریده است.

این فرم ها برای اندازه گیری طیف وسیعی از شاخص های نتایج ، از روش های مختلف جمع آوری اطلاعات تا شامل ارزیابی، مشاهده صنف درسی و مصاحبه شرکای ذینفع بکار گرفته می‌شود. این مجموعه می تواند توسط دیگر سازمان ها استفاده و تغییرات آورده شود ولی با ذکر نام موسسه زنان کانادا برای زنان افغانستان.
